Box 201540, Austin, Texas 78720-1540.  The Texas Public Health Association’s tax identification number is 74-6058233.  Sponsorships and exhibit fees may be paid by check or credit card. 

Exhibit Space:

Each sponsor/exhibitor will be provided with a 6 foot table and 2 chairs. 

Sponsors be sure to send an electronic version of your logo in EPS format to Txpha@aol.com for inclusion in conference marketing materials.
